This certificate verifies that the recipient has met the minimum qualifications for a Community Health Worker through the Work Experience Pathway. By completing at least 2,000 hours of service within the past three years and demonstrating skills and practical training in communication, interpersonal and relationship building, service coordination and navigation, capacity building, advocacy, education and facilitation, individual and community assessment, professional skills and conduct, outreach, evaluation, research, and foundational public health principles, the recipient is certified to provide all covered CHW services, including violence prevention services, in compliance with State of California standards.
